Nursing Services

Community Care Services recognizes there is a strong correlation between mental health and physical health. We use a holistic approach to provide comprehensive services that support both a healthy mind and a healthy body.

We recommend you visit CCS’ nurses once a year for an annual health assessment, as needed or recommended for  wellness checks, and for education and follow up with chronic medical conditions. Treating the whole person with both mental health care and physical health care can lead to better outcomes and can serve as a roadmap toward improving your overall wellness.

What is the difference between an annual assessment and a wellness check?
Annual nursing assessments happen once a year to update your health records. Wellness checks can happen multiple times throughout the year to address concerns as they come up.

What is a wellness check?
During a wellness check a nurse will check your blood pressure, weight, blood sugar level, temperature and discuss any concerns you might have.

How do I schedule an appointment with a nurse?
Call 313-389-7500 to schedule an appointment with a nurse at our Lincoln Park or Taylor locations.

Can I get a walk-in appointment?
When you arrive, let the front desk staff know you’d like to see a nurse, they’ll check to see if our nursing staff can accommodate a same day appointment. If not they will assist you in scheduling an appointment for a later date.

Who is eligible for nursing services?
Nursing services are available to all CCS consumers with a current treatment plan.

Are nursing services covered by insurance?
Nursing services are covered by Medicaid only.
